Having issues with Certification

Hello,
I'm trying to Get Certified on this page:

https://developer.payeezy.com/user/me/edit

When I click the Save button this is returned in Red at the top of the screen.

Company/Legal Entity Name field is required.
Phone #(Office) field is required.
Street field is required.
City field is required.
Postal code field is required.
Country field is required.

I dont see where this information needs to be entered. Can someone please tell me where the fields are located or am I just completely missing something.
